Online Pre-Registration

Sage Portal Online Patient Pre-Registration ensures you always have the patient's most current information. 

Prior to their appointment, patients are asked to go online to respond to appointment type specific questions.  The information provided by the patient can be printed and placed in chart prior to patient arrival.
 

Insurance information can easily be verified for changes.
